The severity scales tend to be hard to measure, and thus a simple, rapid and accessible parameter is needed. Objective: to establish the C-reactive protein/albumin (CRP/albumin) ratio as an early marker of mortality in patients with acute pancreatitis hospitalized at Hospital Luis Vernaza (2018-2019). Design: An analytical retrospective case series. Population: A total of 220 cases of adults with acute pancreatitis who had an albumin and C-reactive protein test on admission to the hospital were included. Data analysis: To determine if the CRP/albumin ratio is an early predictor of mortality, univariate and bivariate analyses were run on the variables, a cutoff point was obtained and the predictive power was determined through logistic regression, according to the statistical significance (p<0.05). Results: The median CRP/albumin ratio was 42.02, and a cutoff point >40 (95% CI 1.20-29.26) was found to be a predictor of mortality, with a p value of 0.029. The average age was 43.47 years, and the female sex predominated with 70% of the cases. The most frequent comorbidity was HTN, with 16.5% of the cases. The mean APACHE II was 6.26, the mean BISAP was 1.43, and the mortality rate was 4.50%. The median hospital stay was 25.2 days, and 15% were admitted to the ICU. There was a positive correlation between APACHE II and the CRP/albumin ratio, with p <0.001, with a similar significance for BISAP. (Acta Med Colomb 2024; 49. DOI:https://doi.org/10.36104/amc.2024.2976).
(Acta Med Colomb 2024; 49. These abnormalities are found in conditions other than hypertension, and their presence is associated with a worse prognosis. Objective: To evaluate dbT lengthening and/or the dbT/jT index in patients with COVID-19 treated in the ICU, as poor prognostic factors. Materials and method: An observational, descriptive, retrospective cross-sectional study. Twenty patients with COVID-19 admitted to the ICU, with an EKG, were included, measuring dbT, calculating the dbT/jT index and relating them to the number of deaths and days of ICU stay. Results: The most appropriate leads for measurement were V2-V3 and V4. A dbT ≥ 93 milliseconds in V2 and V4 was found in 60% of the patients, with a higher number of deaths; a dbT/jT index ≥ 0.40 was found in 45% of the cases and was related to a trend toward more hospital days. Conclusions: Both patients with a dbT ≥ 93 ms and those with a dbT/jT index ≥ 0.40 in the V2-V4 electrocardiographic leads tended to have longer hospitalizations as well as more deaths. (Acta Med Colomb 2024; 49. DOI:https://doi.org/10.36104/amc.2024.2824).
Materials and method: A descriptive, cross-sectional, single-center study. Hypertensive patients over the age of 18 seen in the outpatient or cardiac rehabilitation department at an institution in the city of Valledupar from January 2 to March 31, 2020, were included and interviewed to assess their pharmacological adherence using the Morisky-Green test, and their nonpharmacological adherence through self-report. People with mental disorders rendering them incapable of responding to the interview were excluded. Results: Of the 333 total participants, 127 were considered adherent to pharmacological treatment according to the Morisky-Green test (38.1%, 95% CI: 32.9-42.4%), with no differences found by sex (p=0.66), age (p=0.27), health insurance status (p=0.19), socioeconomic status (p=0.11), marital status (p=0.36), educational level (p=0.19), area of residence (p=0.45), number of medications used (p=0.82), length of treatment in years (p=0.34) or the medical service in which they received care (p=0.66). In addition, only 18.9% (95% CI 14.7-23.1%) adhered to all the nonpharmacological treatment guidelines and 9.6% (95% CI: 6.4-12.7%) adhered to both pharmacological and non-pharmacological treatment. Conclusions: In this report, adherence to antihypertensive treatment was only found in approximately one-third of the patients (38.1%), and therefore multidisciplinary work is needed to educate patients and families, involving them in treatment decisions and improving adherence. (Acta Med Colomb 2024; 49. DOI:https://doi.org/10.36104/amc.2024.2746).
The use of facemasks is believed to potentially decrease the fraction of inspired oxygen and therefore stimulate the production of erythropoietin, thus causing hemoglobin variations. Materials and method: A retrospective observational study was conducted based on an anonymized database to determine if there were variations in hemoglobin levels associated with the use of facemasks. Results: A total of 224,415 complete blood counts drawn between January 2018 and March 2022 were included. The average hemoglobin in all the samples was 13.72 (95%CI 13.72-13.73); the average hemoglobin prior to May 2020 was 13.73 gr/dL and 13.72 after this date, with a statistically significant variation of 0.1 gr/dL (p:0.002). Conclusion: there were significant hemoglobin level variations associated with the use of facemasks during the SARS-CoV2 pandemic isolation. By the same token, these differences are not thought to be clinically relevant today. (Acta Med Colomb 2024; 49. DOI:https://doi.org/10.36104/amc.2024.2967).
(Acta Med Colomb 2024; 49. The idea is to preserve a solemn setting in which authentic empathy for the other person through listening becomes a strong bridge between modern biomedicine and the lived experience of the person seeking assistance. This essay attempts to elucidate, through a concise historical perspective, the need to implement narrative medicine skills within internists' training, which will also promote reflection and self-awareness to complement evidence-based medical practice. Insulinomas are the tumors that most often cause hypoglycemia. However, a variety of both solid and hematological malignancies can cause paraneoplastic hypoglycemia through different pathophysiological mechanisms. We present the clinical case of a patient with symptomatic hypoglycemia in the context of advanced solid organ malignancy. A comprehensive approach was used to ascertain the exact cause of the hypoglycemia and, consequently, determine its treatment and prognosis. (Acta Med Colomb 2024; 49. DOI:https://doi.org/10.36104/amc.2024.3007).
(Acta Med Colomb 2024; 49. Polyglandular autoimmune syndrome type 3 comprises autoimmune thyroid diseases and other autoimmune diseases not including Addison's disease. Up until now, there was only one case report of PAS type 3 in Asia, coupled with isolated reduction of gonadotropin releasing hormone (GnRH) caused by pituitary disease. Case presentation: We present the case of a woman in her 20s with a one-year history of Hashimoto disease and type 1 diabetes, who presented with controlled hypothyroidism and hyperglycemia, but experiencing secondary amenorrhea. Following the GnRH stimulation test, she was diagnosed with secondary amenorrhea attributed to possible autoimmune hypothalamitis and PAS type 3. Our patient's HLA profile reported the DQB1*0201 allele and DRB1*0301 and *0803, DQ2 and DR8 gene sites. Discussion: This is the first report in Latin American literature of PAS type 3 associated with pituitary involvement along with the other two diagnostic elements. A GnRH stimulation test is recommended for patients with PAS coupled with secondary amenorrhea which does not improve despite thyroid disease treatment. This case is anecdotal, with the description of this disease being relevant to our region. (Acta Med Colomb 2024; 49. DOI:https://doi.org/10.36104/amc.2024.3013).
(Acta Med Colomb 2024; 49. She initially had a stable lesion on her right auricle, and then progressed with new lesions on her right buttock, thigh and knee. With the onset of multifocal, recurrent lesions, oral corticosteroid therapy cycles were started, achieving a transient reduction in the size and color of the lesions, but with recurrence when the treatment was stopped. Currently, the patient is stable, having received multiple corticosteroid cycles over the last few years. The association of these two conditions is extremely rare and points to a possible immuno-pathological interrelationship. This case illustrates the difficulties in effective long-term management of the skin lesions associated with Rosai-Dorfman disease, which in this particular case required multiple corticosteroid cycles with a transient benefit. Studies of more sustainable treatments and/or with a better response profile are needed. Communicating these rare cases encourages the exchange of knowledge and experiences among specialists. (Acta Med Colomb 2024; 49. DOI:https://doi.org/10.36104/amc.2024.3035).
(Acta Med Colomb 2024; 49. Although the efficacy of this group of drugs is well established, and it has a favorable risk-benefit ratio, some studies have shown a minimal risk, while others have indicated a clear association, especially with prolonged use of these drugs. Case presentation: A woman in her 70s was being treated with zoledronate 5 mg/year for five years, and over the last three months had been experiencing pain in the proximal third of her right thigh, with 7-8 intensity on the visual analog scale (VAS), exacerbated by exercise and alleviated by analgesics and rest, and which inhibited her walking. During a hydrotherapy session she experienced pain with a VAS intensity of 10 in her right hip, associated with a feeling of instability. On physical exam, she had pain with movement, shortening, abduction and external rotation of her right lower extremity. An x-ray of her right femur reported a displaced, angled, oblique fracture of the proximal third of the femur, and magnetic resonance imaging of the left femur reported marked regular, circumferential cortical thickening, reaching a thickness of up to 8 mm, with no other findings. Osteosynthesis was performed with a cephalomedullary nail along with insertion of a contralateral prophylactic cephalomedullary nail. Comments: The current literature suggests that the risk of atypical hip and femur fractures increases with prolonged bisphosphonate use and decreases after stopping the drug, with persistent risk for up to four years. Although the pharmacological and surgical treatment is debated, careful semiology helps clinicians to suspect the condition and perform confirmatory tests. Joint orthogeriatric assessment will help in making decisions and determining the pertinence of prophylactic interventions like the one described in this case. (Acta Med Colomb 2024; 49. DOI:https://doi.org/10.36104/amc.2024.3042).
