Health services systems and markets have as a priority in their policy agendas the issue of improving the performance and quality of health services through the incorporation of incentives to the stakeholders. This work answers the research question on how to transfer incentives to different agents of the health system in Colombia, with particular emphasis on health service providers, to improve their efficiency, in terms of the quality of health services, minimizing the negative externalities. To answer this concern, a realist systematic review is carried out with the extraction of information from 42 studies. An iterative and process-based logic model is used. As a result, six major intervention mechanisms with different intensity levels have been identified that deal with the transfer of incentives, specifically to health service providers, taking into account externalities and their effects on quality and efficiency.
