Abstract

Objectives: To elaborate and to validate a guide of handling the pediatric patient with full stomach. Specific objectives: - To define and to approve the model - To enable the human resources - To elaborate and to validate the guide - To manage the implementation Methodology: A systematic revision of medical literature was made; the steps followed were: conformation of a work group, qualification of the human resources, elaboration of the clinical questions, search of the literature, elaboration of the guide, and validation by peers. Results: After making a complete revision of the collected information, and having made and analyzed a survey to anesthesiologists of the country, a series of recommendations occurs at induction: the semifowler suggests itself like better position, to use the maneuver of Sellick, positioning of preoperative naso or orogastric tube in patients with high liquid gastric content, to use sounding 14 french, prefering the technique of fast sequence by intravenous route with cuffed orotraqueal intubation; using succinilcolina, and like alternative, rocuronio to greater or equal doses at 0,9 mg/kg; if the venous access is difficult, it continues trying it (considers intrabony), way or makes inhalatory technique; use farmacológical measures to diminish the broncoaspiration risk; estube the patient in a awake fashion. Conclusions: 1. GPC are useful tools for taking decisions. 2. In pediatric anesthesiology, clinical evidence is scarce. 3. Anesthesiologists must make important decisions in the professional exercise.
