Resumen

ESCOBAR, María Fernanda et al. Surgery of control of damages: an applicable concept in gynecology and obstetrics. Colomb. Med. [online]. 2005, vol.36, n.2, pp.110-114. ISSN 1657-9534.

OBJECTIVE: Reviewing cases of damage-control surgeries in gyneco-obstetric patients in Hospital Universitario del Valle, Cali, Colombia between 1997 and 2002. TYPE OF STUDY: Cases series which include 28 gyneco-obstetric patients. The analysis of quantitative variables was carried out by non-parametric methods and the categorical variables by frequencies distribution. RESULTS: The age rank was between 13 and 52 years old, five patients were primiparas and 18 had had at least 1 cesarean. The initial indicators for surgery were: abruptio placentae, uterine atonia, placenta previa and Hellp syndrome. In all the cases packing and non-conventional closing surgeries were practiced. Damage control was identified in the initial surgery on 16 patients. Ten patients who had more than 12 APACHE points (Acute Physiology and Chronic Health Evaluation II) died due to multiple organic dysfunction. CONCLUSION: Damage control allowed the hemorrhage control in all the cases. Delaying in making the decision, greater physiological deterioration and developing of the multiple organic dysfunctions correlated with greater possibility of dying.
