Clinical errors in healthcare can be fatal in some patients. They occur because the complexity of healthcare. Government, academic, human and institutional factors are involved in its cause. The creation of organizational culture on patient safety at hospitals and implementation of safe practices will produce safer clinical acts taking away daily errors from healthcare institutions.
