The frequency of blood transfusion remains high during cardiac surgery. The mandatory hemodilution resulting from crystalloid priming of the cardiopulmonary bypass circuit represents the major risk factor. There are methods to lower blood requirements (acute normovolemic hemodilution, eritropyetin and retrograde autologous priming RAP). We propose that retrograde autologous priming of the bypass circuit would result in decreased hemodilution and red cell transfusion; it is safe and cheaper technique. Eighty patients undergoing elective cardiac surgery were allocated to retrograde autologous priming and 80 patients were managed with the conventional technique. The RAP group had a 39.31% less of blood transfusion. 37 of 160 patients did not receive any homologous red cell transfusion during the whole. hospitalization. 64.9% of the RAP group and 35.1% of the control group. (p= 0.039). These data suggest that Retrograde Autologous Priming decreases significantly the requirements of transfusion during cardiac operations.
