OBJECTIVE: to determine the relationship between the level of development of the competences of the managers of health services in Antioquia as well as the quality of the institution they conduct. METHODOLOGY: a probabilistic stratified random sampling by level of complexity was done and then a simple random sampling in each one. 112 managers constituted the sample. Four instruments were used; three of them to evaluate the competence level of the managers and the other one to evaluate the level of institutional quality. RESULTS: the development of the competences among managers was found between the basic and inexperienced levels, showing a poor performance in their management tasks. Additionally, a high correlation among competences was also identified indicating that the high or low level of development of a competence directly affects the performance of the manager. The quality of the institutions showed levels of business development between incipient and medium, thus revealing some sort of functional units which are determinant in the business development, such as service operations, planning, management control, and marketing. Finally, it was shown that the presence of some competences in the manager positively impacts the development of the functional units. Thus the performance of managers has direct effects on the success or failure of the institution.
