A case of a 51 year-old male is presented. He had a large paraesophageal hernia with totally intra thoracic stomach and organoaxial volvulus. The classification, symptoms, diagnosis, technical complications and surgical correction by laparoscopic way are discussed. A new group, the Type V, is proposed. In this case was necessary to open the abdomen. Literature review is made.
