Background: the consequences of otitis media with effusion in audition, cognitive and linguistic development are very strong sense for an accuracy diagnosis and effective treatment of this disease in children. Objective: to compare the operative performance of pneumatic otoscopy and tympanometry as diagnostic tools in otitis media with effusion. Materials and methods: diagnostic test study, prospective registry of patients who underwent tympanocentesis and tympanostomy tubes in the Hospital de la Misericordia from january 2002 to december 2003. Pneumatic otoscopy in the preoperative visit and tympanometry nearest to the surgical procedure. Results. Sixty patients, age average 39.6 months. 55% of patients were men, and 18.3% have cleft lip and palate. The sensibility of pneumatic otoscopy was 90%, specificity of 78.6%, for tympanometry 75.8% and 71.4% respectively. The positive predictive value of pneumatic otoscopy was 93%, the negative predictive value was 71%, and 90% and 48% for the tympanometry respectively. The positive likelihood ratio for the pneumatic otoscopy was 4.2 and the negative likelihood ratio was 0.12, for the tympanometry were 2.65 and 0.34 respectively. The diagnostic odds ratio for the pneumatic otoscopy was 35 and 7.7 for the tympanometry. Conclusion. Indicators showed a superior acting of pneumatic otoscopy for the diagnosis of otitis media with effusion.
