Myocarditis is an entity caused mainly by viral agents. Less frequent etiologies include other infectious agents, autoimmune diseases and toxins. Fulminant myocarditis is an unusual complication with a rapid progressive course resulting in severe heart failure and cardiogenic shock. Continuing, we present the case of a previously healthy young patient who presented with Stokes Adams' syndrome upon admissions to the emergency department and whose clinical course subsequently progressed to acute heart failure and cardiogenic shock. Diagnostic confirmation of fulminant myocarditis was made by myocardial biopsy. Advanced hemodynamic monitoring, vasopressor and inotropic management and mechanical ventilatory support measures were needed with a satisfactory clinical outcome, though, requiring definitive bicameral pacing.
