General anesthesia is the preferred method for videolaparoscopic surgery; techniques of regional anesthesia are still under scrutiny and deserve further investigation. We concluded a descriptive, prospective and longitudinal study in 136 patients that underwent videolaparoscopic surgery; in 68 we utilized epidural anesthesia (group I) and in the 68 remaining patients general anesthesia (group II). Results were analyzed in both groups, demonstrating the viability of regional epidural anesthesia for laparoscopic cholecystectomy and other laparoscopic surgical procedures. The use of this technique demonstrated lesser variations in the measured parameters: concentration of expired C02, minimal inspiration of C02, respiratory rate, mean blood pressure and cardiac rate, as compared with the general anesthesia technique before, during and after the creation of pneumoperitoneum; changes from basal values occurred between the onset and 20 minutes after induction of pneumoperitoneum. Shoulder pain during the procedure was incremented in a directly proportional manner to the pneumoperitoneum pressure in the awake patient and was successfully controlled with small doses of phentamil.
