Like in other countries the practice of obstetrics and gynecology has a medical legal high risk. The information and acceptation of the written informed consent is mandatory by law in Colombia but many of us ignore the immense value for a legal medical defense. The aim of this work is to propose a form of model for each surgical or medical act of informal consent according to protect the patient autonomy and make easier and more secure the medical practice.
