Objective To know the effectiveness and the security of the sublingual misoprostol in third stage of labor. Materials and methods In a double-masked placebo randomized controlled trial, during vaginal delivery women were randomly assigned to receive a dose of 100 µg sublingual misoprostol or placebo in third stage of labor, immediately after cord clamping. Blood loss were measured, duration of the third stage of labor, and side effects. Results Blood loss in the misoprostol group was minor 599 + 426,9 mL vs. 670,3 + 482,5 mL (p>0,05) in the placebo group. Without statistically difference between groups. Duration of the third stage of labor was minor in misoprostol group 431 + 231,1 seg vs. 517,6 + 412,7 seg (p > 0,05) in the placebo group, without statistically difference. No predominance of side effects. Conclusion Use of 100 µg of sublingual misoprostol in the third stage of labor was well tolerated and associated with a statistically no significant trend toward less blood loss and less duration of the third stage of labor.
