Introduction: The onset of psychosis over the age of sixty has aroused diagnostic controversies since the beginning of psychiatry. One of the major discussions has centered on the diagnosis of schizophrenia in that age range. Nowadays, there exists an international consensus on diagnosis, nomenclature, and treatment of late and very late onset schizophrenia. Method: Case report. Result: The authors present a case of a woman who at the age of 64, started to suffer delusions and hallucinations and who was diagnosed with schizophreniaparanoid type, after the authors had made the differential diagnosis with delirium, other somatic conditions, dementia, delusional disorder, and affective disorders. Discussion and conclusion: The historical validity of late onset schizophrenia, its differential diagnosis, and clinical features are discussed.
