The acute lobar nephronia or acute focal nephritis is a rare entity in children. It is a kidney infection with varied and unusual clinical presentation. It is diagnosed through computerized axial tomography (CAT), which is currently considered the most sensitive and specific test. Appropriate antibiotic treatment, according to the isolated germ, is the key to successful treatment that prevents progression to renal abscess. This article presents the case of a pediatric patient who was admitted with fever of prolonged origin and, after several days of inpatient study, was diagnosed with acute lobar nephronia.
