Introduction:
The safety of low molecular weight heparin (LMWH) prophylaxis in patients with impaired kidney function remains undefined.
Objective:
To establish if the reduction of the glomerular filtration rate (GFR) is associated with the development of bleeding under LMWH prophylaxis.
Materials and methods:
A cohort of non-surgical patients with LMWH prophylaxis was constructed in order to conduct a case-control study. Monitoring allowed to find bleeding cases, while four controls were chosen from the population at risk at the time of bleeding onset.
Results:
Out of 716 monitored patients, 51 presented with bleeding, with an incidence of 3 cases per 100 patients per day of treatment; 39% of the cases presented with major bleeding and 204 controls were chosen. The multivariate analysis showed no relationship between bleeding and abnormal renal function due to GFR reduction. However, only 3.5% ofpatients had GFR <30ml/min. The final adjusted odds ratio (OR) was 1.27 and the 95% confidence interval (CI) was 0.60-2.68.
Conclusion:
No correlation was found between the reduction of mild to moderate GFR and the development of bleeding under LMWH prophylaxis.
