Abstract

Objective:To determine the risk factors associated with mortality in the high risk surgical patients hospitalized in the critical care unit. Materials and Methods:An observational descriptive and retrospective study, was made including 974 high risk surgical patients during five years in the post surgical critical care units (PSCCU) of the Hospital Militar Central and Hospital Universitario Clinica San Rafael, from Bogotá, . All the variables which measure the evolution in the post surgical period were analyzed in the two PSCCU, and a logistic regression univariate and multivariate analysis were made, determining the risk calculation, with the corresponding confidence intervals, to define the variables associated with the highest mortality risk. Results: The age, the severity of the patient calculated by APACHE II score, the coexistence of comorbidity specially COPD, and beeing a patient from the surgical department, were the independent variables constituting risk factors for the mortality in the post surgical period in these the critical care units. The diagnosis of sepsis was associated with the highest mortality risk (OR 4, 4), compared with the others independent variables. Conclusions:Variables considered in this study as addominal sepsis, COPD, patients from the surgical departament and apache II score more than 14, were conclusive to explain the final outcome, so defining the mortality risk factors in the post surgical period in the high risk surgical patients in our critical care units.
