Abstract

Objective:To perform an evidence based algorithm on the use of ultrasound in placing central venous catheters. Methods:A systematic review of literature was performed on the following data bases: Pubmed, Cochrane, HINARI, OVID and Lilacs. Studies with a proper design were included, randomized controlled trials, metanálisis and systematic reviews were considered. Studies with methodological flaws including observational studies, case reports and letters to the editor were not considered. Results:We present an algorithm for a safe placing of central venous catheters in hospitals of our Country where this resources exist, the algorithm is composed of seven steps to follow, it initiates with the clinical question of indications for central venous catheter placing, goes through the steps of the procedure until the successful cannulation with out ultrasound. Its use is proposed if exists any risk factor or if more than 3 attempts are made. Conclusions:After evaluating the evidence, we suggest the use of ultrasound guided central venous cannulation in patients who present risk factors, to prevent complications, reducing the time of insertion and decreasing morbility related to the procedure.
