Peribulbar blockade is an anesthetic technique which consists in applying a local anesthetic outside the muscular layer of the orbit. The different levels of systemic toxicity of the local anesthetics currently used, has pushed the search of new agents like ropivacain. A 1. Pseudorandomized clinical trial, triple blind was designed. 2. The objective was to compare the differences in quality of anesthesia of the traditional mixture of local anesthetics and ropivacaine. Aquinesia, intraocular pressure and analgesia were evaluated. 103 patients operated of extracapsular extraction of cataract and insertion of intraocular lens, reervided between November and December 2003 at the Rafael Uribe Uribe clinic in Cali, Colombia. The patients were randomly assigned to two groups: Group 1: Ropivacain 0.75% 10 cc and group 2: Bupivacain 0.75% 5 cc. plus lidocain 2% 5cc. plus hyaluronidase 30 Units (traditional mixture). Groups were similar in demographic variables (sex, gender, race), with a mean age of 70 years. Starting at the mean range and the sum of ranges we evaluated: aquinesia, were group 2 showed more immobility than group 1 (p=0.008) and, pain through the analogue visual scale without difference between the two groups (p=0.075). The intraocular pressure before the blockade was analyzed starting at the mean, which was for the both groups 14.1 mm Hg. (p=0.0784). The intraocular pressure after the blockade was less for both groups, mean for group 1 was 10.8 mm Hg. And for group 2: 10.4 mm Hg. (p=0.409) In conclusion both anesthetic solutions provided excellent surgical conditions in peribulbar blockade .
