Over the course of the last few years it has become increasingly interesting to have a practice in medicine that would be cost effective. The fast advance in techniques, drugs and anesthetic monitoring has allowed us to improve the way that anesthesia is currently practiced; however, we do not have an ideal anesthetic technique for outpatient surgery. We carry out a clinical trial comparing the recovery time and cost of the different anesthetic technique commonly used in our institution for maintenance. The main objective was to have a cost analysis to be presented to our pharmacy administration to justify the introduction of new anesthetic agents in our institution.  We hypothesize that the recovery time will be decrease by half with sevoflurane or desflurane and that the increased cost of the new agents will be offset by the decreased recovery time. We studied seventy patients scheduled for ambulatory surgery, with ages between 18 and 65 years and with an ASA PS I or II. After anesthesia, we evaluated time until the ocular opening, post anesthesia time until the patient was oriented on time and space, recent and remote memory as well as any adverse events, side effects and the cost of each of the therapies. We did obtain a significantly less time to full recovery with desflurane or sevoflurane combined with remifentanyl compared to isoflurane alone or in combination. However, the cost analysis was still higher with the new evaluated anesthetic agents than with isoflurane alone or combined with remifentanyl or nitrous oxide.
