Ankylosing Spondilitys is an uncommon reumathologic entity, defined as an inflammatory arthritis of the spine always involving the sacroiliac joints. Frequently inflammation and ankylosing also affect peripheral articulations, and these patients need mayor orthopaedic surgeries like total knee replacement, since their daily activity is limited by the joint disease. We present the case of a young woman patient with advance stage of this patology, who required this quirurgic procedure, and therefore our anesthetic intervention in order to perform it. The preanesthetic evaluation showed true predictors to difficult in the management of the airway and to apply regional anesthesia techniques. She was unable to lie flat. We resolved to perform the procedure under general anesthesia, after intubation with fibroscopy because of the peculiarity of the airway. We made use of the opportunity, to discuss features and complications of the disease which can lead to perioperative difficulties, the matter of a preanaesthetic visit, and the different proposal modalities in the literature for the anesthetic management of these patients.
