Anticoagulation and antiplatelet therapy of acute coronary syndrome can result in severe hemorrhagic complications. A complication that can go unnoticed is diffuse alveolar hemorrhage, which causes acute respiratory failure and is life-threatening.

We report two cases of acute coronary syndrome with complications of alveolar hemorrhage.

Predisposing factors, diagnosis and treatment for this condition are analysed.
