Venous thromboembolism is a relatively common condition that is identified in the emergency services and represents significant morbidity and mortality in the population. Its most severe outcome is pulmonary thromboembolism, which triggers several pathophysiological processes that can end in cardiac arrest and subsequent death of the patient. A thrombus found in the right cavities or in the vena cava is an undetectable process, this condition is known like ongoing thrombus. The finding of thrombus in transit has been associated with high hospital mortality. We present the case of a 64-year-old female patient with a history of active uterine cancer, who was hospitalized for deep vein thrombosis and in whom a control transthoracic echocardiogram is taken, showing evidence of a highly mobile thrombus in transit between the right atrium and inferior vena cava compatible with ongoing thrombus at this level.
