Hypertensive crisis is defined as elevation of blood pressure above 180/120 mmHg. There are two presentations: hypertensive emergency, characterized by acute and severe target organ damage, and hypertensive urgency, when this damage is absent. Hypertensive emergency has a high rate of morbidity and mortality, for this reason prompt initiation of effective treatment and monitoring in the intensive care unit are mandatory. Instead, blood pressure reduction in hypertensive urgency should be cautious and can be completed on an outpatient basis. As heterogeneous group of hypertensive disorders, it is common to make mistakes during the patient care, mainly in the emergency department. This article reviews the hits and mistakes about diagnosis and treatment of hypertensive crisis.
