ABSTRACT Introduction: street people are three times more likely to die compared to the general population, which is reflected in the high risk of a wide range of health problems such as HIV. This research aims to identify condom use as an indicator of vulnerability and social and demographic contexts, which may be associated with this practice. Methods: This is a descriptive sectional study. Information was obtained on some characteristics of the demographic, social, economic, risk behaviors, and HIV seroprevalence of street dwellers in the second half of 2011 in the city of Medellin, Colombia. The data analysis and statistical tests include calculating OR with a significance level of 5%. Results: The average age of street dwellers was 38.2 ± 10.9 years and the most common of 34 years. 67.9% were male. With regard to condom use as a factor of vulnerability to HIV, 10.1% of street dwellers used a condom with couples who considered stable, 30.7% used it with casual partners and 21.2% with commercial partners. 90.7% of the street people men did not use condoms with casual partners. Significant differences in condom use by marital status in stable couples and age groups were found in commercial partners. Conclusions: Inconsistent use of condoms, the failure to adopt safe sex practices and unfavorable conditions in the social and economic environment of street dwellers are situations that make them vulnerable to HIV and require the implementation of priority policies in this population at risk
