Elderly correspond to a population with special characteristics, which becomes them in a diagnostic and therapeutic challenge, in the approach to headaches. If it is true, the primary headaches are those that present more frequently in this group, reaching up to 66 %; however, within the approach, it is also necessary to know about the main secondary headaches that can occur and especially what represents a risk or endangers the patient's life. This is why we made a review of available literature about this issue, in order to offer an updated and easily accessible guide for all. The chapter begin with a clinical case designed by the authors with the respective discussion, we present some epidemiological data , furthermore a description of the most frequent primary, secondary headaches and neuralgias in older adults; finally we mention some headaches not so frequent but that must be considered and therapeutic aspects.
