Abstract
Caracterización de la función hepática en un grupo de pacientes con dengue en el departamento del Meta. Rev Col Gastroenterol [online]. 2004, vol.19, n.3, pp.171-177. ISSN 0120-9957.
Objectives. This paper describes in patients with dengue fever or dengue hemorrhagic fever, the clinical findings and laboratory pattern of liver function. Well study if those abnormal levels differ between dengue fever and dengue hemorrhagic fever or use to show other non dengue fever. Methods. Design is cross sectional study in order to find pattern of liver involvement (AST, ALT, Bilirubins, Alkaline Phosphatase) in patients with dengue fever and dengue hemorrhagic fever, and explore association among that laboratory test. Results. We studied 32 patients who had classic dengue fever and serology IgM anti- dengue positive. Dengue hemorrhagic fever:13 (35%). Classic dengue fever: 19 (51%). Non dengue- serology negative: 5 (14 %). Female 18 (56%), male 14 (44%). The most common clinical findings were: fever 32 (100 %), headache 24 (75%), vomiting 22 (69%), bleeding 4 (13%).There were no clinical findings of liver disease. The laboratory pattern of liver function was: Bilirubin: mean 0.56 mg/dL (0.44-0.68), Direct Bilirubin: mean 0.27 mg/dL (0.20-0.34); Indirect Bilirubin: mean 0.31 (0.20-0.42); AST: mean 165 U/L (101-229); ALT: mean 157 (78-236); ALP: mean 300 (254-346). The Direct Bilirubin >0.27 can able to identify the infection as dengue hemorrhagic fever or classic dengue fever, OR = 3.50, Sensibility: 53.8 %, Specificity: 94.7 % and VPP =87.5%. The Direct Bilirubin: >0.27 mg /dL also can able to identify the infection as dengue fever or other fever causes no dengue. OR= 2.74, Sensibility: 40.6, Specificity: 80 %, VPP: 92.9 % Conclusions. The impact in liver function in patients with dengue was more important in dengue hemorrhagic fever, however liver profile showed that liver dysfunction was mildly elevated. The Direct Bilirubin should be useful to identify the infection as Dengue Hemorrhagic Fever or classic Fever.
