Abstract

MEJIA, Andrés et al. Palliation of a malignant gastroduodenal obstruction. Rev Col Gastroenterol [online]. 2006, vol.21, n.1, pp.19-21. ISSN 0120-9957.

Main objective: The Gastroduodenal outlet obstruction is one of the most common symptoms of malignant gastroduodenal. In the past the only therapeutic option was surgical gastroyeyunostomy which is associated with a high rate of morbi-mortality. The development of stents has been changing this perspective. This retrospective study compares two groups one with gastroyeyunostomy and another with stents, having as outcomes: technical success, clinical success, morbid-mortality and days of hospitalization. Methods: Clinical data from 30 patients history were recollected from January 2003 to May 2005. One group (15 patients) had gastroyeyunostomy, another group (15 patients) had stents. Results: The groups were similarly in demographic data. Technical success did not showed differences but clinical success was lower in the surgical group (87% stent vs. 56% surgical). Days of hospitalization were less in the prothesis group (3.4 days vs. 7.5 days). The surgical group had a morbility as high as 40%. Conclusions: This retrospective study shows that the use of stents in the palliation of gastroduodenal outlet obstruction may have better clinical success and lower rates of morbi-mortality than the surgical approach. So we think that randomized clinical trials are needed to support these findings.
