The anemia is a frequent complication of the intestinal inflammatory bowel disease (IBD) seen until in a third of the patients. With diverse economic and clinical repercussions, the anemia affects the quality of life primarily comparable to that of the patients with cancer. The major causes are: ferropenic anemia and anemia of the chronic disease. For the frequent coexistence of both, it is necessary the use of ferrocinetic studies to direct the therapy, which goes in most from the cases to the iron replacement. Although at this moment an intense debate exists about the security for the use of the oral iron, the parenteral iron in the form of sacarate has shown its security in the cases with severe anemia (Hb < 10). The case of a patient is described with severe anemia, managed with iron sacarato with functional improvement independent of the correction of the anemia.
