Fecal stream diversion is the cause of inflammatory bowel disease secondary to the absence of luminal short chain fatty acids that provide nutrients to the colonocyte. The endoscopic findings include edema, erythema, and friability. Ulceration and nodularity could appear. The pathology findings are variable, and depend on the pre-existing disease. The inflammatory changes become apparent three months after surgical diversion. The patient can appear asymptomatic, and progressively abdominal pain, mucoid and bloody discharge are the subsequent manifestations. Differential diagnosis includes any chronic colitis, ulcerative colitis, and Crohn’s disease, and the fist one is the previous disease. The treatment frequently offered is short chain fatty acids by rectal enemas, but trial results are contradictory.
