Introduction:
The liaison nurse is not of recent origin, their functions have been diversified and have positioned themselves as important professionals in the hospital discharge process. The objective of this review is to identify the role of the liaison nurse in the continuity of care for patients after hospital discharge.
Methodology:
An integrative review carried out in four databases, with publications between 2010 and 2020, 12 studies were selected. The following descriptors were used for the search: patient discharge, continuity of patient care, transitional care, case managers, work profile and professional practice.
Results:
The liaison nurse acts towards discharge planning, resource management and health system connection.
Conclusions:
Liaison nurses work towards a patient-centered care model, avoiding fragmented care, improving the transfer of information between levels of care and promoting continuity.
