Abstract

The objective of this study was to determine the validity of the markers of severity like APACHE II, score of Ranson and classification of Balthazar in the prediction of mortality in the patients with acute pancreatitis. Historical descriptive observational study of cohorte with base in the registries of the patients with diagnosis of acute pancreatitis from November of 1999 to November of the 2005, that entered the surgical units of intensive care of the Hospital Universitario Clínica San Rafael and Hospital Militar Central. We analyze the information of 57 patients being a mortality of 19,3%, APACHE II in relation to mortality showed an area under the curve of 0,847 with a I.C. 95% (0,7415 - 0,9541) superior to the Ranson to the 48 hours with an area under the curve of a 0,687 and IC 95% of (0,5518 to 0,8085); was not an association with the score of Ranson to the entrance nor with the classification of Balthazar, curves ROC for APACHE II and Ranson to the 48 hours did not show to differences in their areas, being a greater tendency in the validity of the scale of APACHE II. In the present study the markers that showed a greater validity in the prediction of mortality were the score of APACHE II and the score of Ranson to the 48 hours, being Apache II the one that both showed one better tendency the discrimination without significant differences between scores
