Penetrating gunshot wounds of the Central Nervous System have been for years a clinical challenge. Although in recent years both the technological advances and the better acknowledgement of the physiopathology of this type of wounds, there is still ample controversy in its clinical management due to their collateral complications and poor clinical outcome. Unfortunately war conflicts and easy availability of weapons in the market make this type of lesions frequent everyday burden for general physicians practice. In lieu to contribute to improve continued education in this matter, a detailed historical review of penetrating injuries to the central nervous system is presented. In addition, basic forensic ballistics concepts to better understand the physiopathology of these lesions are also discussed here. More detailed mechanistic concepts will be discussed later on.
