A diagnostic aid methodology of cardiac dynamics evaluated in Holter based on probability and Boltzmann Gibbs entropy ratios was developed, whose clinical usefulness has been demonstrated in the evaluation of normal, diseased and evolving dynamics. This methodology was applied in thiswork using a blinded study in which the findings of 35 Holter, 7 normal and 28 with a diagnosis of arrhythmia treated with and without Metoprolol, were masked. Attractors were constructed from the maximum and minimum heart rates, and total beats/hour, and the probability, entropy and entropy ratios of consecutive pairs of heart rates values were calculated. Subsequently, sensitivity, specificity and Kappa coefficient were calculated to evaluate the diagnostic concordance between the mathematical methodology and conventional clinical diagnosis. It was shown that the methodology difference normal dynamics from the arrhythmic ones; the sensitivity and specificity values were 100% and the Kappa coefficient was 1, showing that it is also possible to establish quantitatively the degree of sharpening. Analysis of the entropy proportions showed that 77.77% of the dynamics treated with Metoprolol have mathematical characteristics of acute illness, showing that the quantification of these acute states shows how far they are from normality. The diagnostic ability of the methodology was confirmed to differentiate normal and arrhythmic dynamics; these last one treated with or without Metoprolol, allowing to quantify the evolution of each dynamic towards acute illness.
