Resistance to anti-tuberculosis drugs is becoming increasingly frequent and currently affects the general population worldwide, compromising both new cases of tuberculosis and those previously treated, requiring specific treatment schemes with several second-line drugs for a long period of time to achieve bacteriological conversion, making it a challenge for treating physicians as these drugs generate many adverse effects that negatively impact adherence to treatment. The following is the case of an adolescent girl diagnosed with multidrug-resistant tuberculosis, which was managed according to national guidelines with two group A drugs and two group B drugs (due to the unavailability of bedaquiline), proving negative resistance to second-line drugs and establishing adequate adherence and tolerance to treatment and, importantly, to follow-up.
