This article presents some historiographical elements of the psychiatric institutions in the country, which were significant in the process of modernization of mental illness care in the first half of the twentieth century. The aim is to characterize both psychiatric establishments and expert agents, as producers of the modernization of public health in Colombia. To this end, a series of national investigations developed with various sources from the psychiatric field was reviewed, which show the contradictions between the real state of the mental hospitals and the projects for institutional change. The historiographical elements, presented in the first half of the twentieth century, there is a strong tension between the Assyrian logic -rooted in the moral therapy of the confinement and the specialized psychiatric logic- that sought to translate abnormal symptoms with the language of medicalization, within the framework of the new nosological discourses of diagnostic and statistical manuals on mental disorders.
