This article addresses the concept of intelligence failure, its features, and its main causes from a psychosocial perspective. From this point of view, it argues that errors are inherent to all human activity and the nature of intelligence. Therefore, their occurrence is normal and inevitable in complex systems, such as intelligence services, agencies, and communities. However, it contends that, although they cannot be avoided, it is possible to minimize their occurrence as much as possible and mitigate the negative impact they could have. In this sense, some measures and actions that can contribute to this end are proposed.
