Abstract

This article aims to contribute to the debate on the current direction of the process of psychiatric reform in Brazil, presenting a national panorama of the advances and challenges faced. In so doing, it reveals the coexistence of two paradigms of care, one based on the institution of the mental health facility and the other focused on psychosocial care. The parallels between the two directions create severe tensions in the way the services of care are given, and simultaneously uncover the vertical character of psychiatric knowledge, the hegemonic attributes of certain specialties, and the fragmentation of the process of work. A review of these tensions and characteristics allows for the identification of which specific aspects of the Brazilian model of reform require strengthening as well as increased investment.
