Subdural empyema accounts for approximately 20% of localized intracranial infections, and may be difficult to diagnose due to its unspecific presentation, among these, central nervous system infections due to Actinomyces are rare. The objective of this report is to present a clinical case of a patient who was diagnosed with actinomycotic subdural empyema. The patient required surgical management and the case was confirmed through histopathological studies, subsequently he was managed with antibiotic treatment and at the end of the therapy he showed clinical improvement, recovering completely his normal function responding satisfactorily to the established treatment, and continued in ambulatory follow-up where no neurological or paraclinical deterioration was evidenced.
