Pulmonary thromboembolism is a clinical condition linked to various risk factors and high morbidity. The scales aimed at establishing a pre-test probability are designed to detect risk factors for presenting the disease, however their diagnosis is a challenge when there are no such factors, as in the report of this case attended in a fourth-level hospital in Bogotá (Colombia). The 2019 European Society of Cardiology Pulmonary Thromboembolism guide was based on the pulmonary thromboembolism guide and a search was conducted using the terms MeSH: pulmonary thromboembolism in low probability patients in Pubmed, Google Schoolar, and Scopus sources, with insufficient results which makes it important to report this case and leads to highlighting the impact of clinical presumption as a fundamental tool for diagnosis in the emergency.
