We report a series of patients in whom correction of ventral hernia consequent to laparostomy was performed by means of a technique of separation of the abdominal wall components at Hospital Universitario de la Samaritana (Bogotá, Colombia) in the period June 2001 to december 2002. The technique is simple; it includes relaxation incisions and the construction of fasciomuscular flaps for the correction of large defects of the abdominal wall; this technique avoids the use of a prosthesis. Defects measuring 14-18 cm in diameter and 20-26 cm in length were corrected; in only one case it became necessary to use a prosthesis to completely cover the defect. Recurrence occurred in one patient that presented and abdominal tumor, wich may be explained by a rise in intraabdominal pressure. There were no changes in airway resistance or in the intraabdominal pressure. One patient died following the development of abdominal sepsis resulting from dehiscence of a colo-colonic anastomosis performed concomitantly; minor complications included some seromas and superficial infection of the operative site. In conclusion, we consider this technique useful in selected cases, with advantages over the routine use of mesh prosthesis and its associated potential complications.
