In the period April 1st, 1999, and march 31st, 2001, 732 appendectomies were performed in pattients older than 15 years with the diagnosis of acute appendicitis at Hospital Universitario Clínica San Rafael, Bogotá, Colombia. This is a retrospective study of 100 cases operated on before the introduction of a sulbactam/ampicillin protocol, and of 100 cases after the introduction of such protocol, with the purpose of analyzing results in the two groups, including the incidence of OSI ( Operative Site Infection), hospital stay, doses, combinations, and costs of the antibiotics utilized. Our data indicate that there are no significant differences in hospital stay (1.58 vs. 1.72 days), incidence of OSI (3% vs. 4%) in the two groups, but that the use of antibiotics under protocol results in reduction of costs (Pesos col 45,000 vs Pesos col 82,587), number of doses (1 vs. 2.95), combinations and numbers of antibiotics ( 1 vs. 1,19). Therefore, the rational and adequate use of antibiotic prophylaxis results in benefit both to the patient and to the institution.
