Even though studies abound regarding hepatic regeneration, the literature coincides in that this thematic needs to be further explored in pursuit of new answers to explain the responsive behavior to stimuli such as surgical resection, injury, and loss of volume, both in the remaining hepatic tissue in the living donor and in the hepatic graft in the recipient. In order for the regeneration process to take place, it is necessary to develop diverse cellular events, including a double cellular system composed by a reserve compartment and the hepatocytes. In the live donor, according to the grand regenerative capacity of this organ, it can be assumed that regeneration occurs rapidly to achieve complete volume replacement; nevertheless, donors do not tend to recuperate the initial liver volume, in spite of complete recovery of normal hepatic function. Diverse factors intervene in the recipient of a living donor for an adequate regeneration, such as time of ischemia, size of the graft, immune response, reperfusion, and the transplanted lobe; if all this occurs favorably, function is restored, with positive impact on the survival rates and the patients’ quality of life.
