Internal abdominal hernias are a rare cause of intestinal obstruction due to the sliding of intra-abdominal organs, especially small intestine, through congenital or acquired defects in the mesentery or intraperitoneal spaces. Internal supravesical hernia is one of the less frequent presentations of this entity, and its diagnosis in most cases is intraoperative.

We present the case of a male patient who consulted with manifestations of an intestinal obstruction, taken to emergency surgery with the finding of an internal supravesical hernia.
