In accordance with the publishing direction we proposed at the beginning of our term of office, in this issue we present two additional sections. Neither are innovations in the field of medical and scientific publications, but we hope that they are interesting and entertaining for our readers. First, Clinical Problems tries to develop the well-known format of examining a real clinical case which serves as a guide, allowing an expert to share her or his approach to a particular problem or issue, looking at the questions that we as clinical practitioners face in similar cases in our daily practice.

To start this section off we invited Dr. Diego Aponte to demonstrate the appropriate approach to a patient with an incidental pancreatic cyst. This is a situation for which there is increasing advocacy of solutions requiring a proliferation of abdominal imaging of every imaginable type.

The second new section is the Diagnostic Image. This section presents endoscopic or radiological images illustrating infrequently encountered presentations of common pathologies or rarely encountered illnesses that present well-known symptoms.

Here, we would like to issue a cordial challenge to our readers to send us material for these sections. Send us your feedback, comments and notes. Please be assured that your contributions are very valuable to us as a guide for mapping the course of our publication.
