Objetivo: evaluar las diferentes opciones de tratamiento que eliminan el fluido del hidrosálpinxantes de realizar una fertilización in vitro (FIV). Metodología: se realizó una selección y evaluación de la literatura relacionada con el tema, obtenida de las bases de datos de MEDLINE y EMBASE de enero de 1997 a septiembre de 2007. Los artículos fueron evaluados con relación a su objetivo, su metodología, tipo de estudio y tipo de revisiones. Resultados: la salpingectomía previa a la fertilización in vitro (FIV) es el único método que ha probado ser efectivo en restaurar las tasas de nacimientos en pacientes con hidrosálpinx. La ligadura proximal de la tuba uterina también puede ser efectiva, de acuerdo con un estudio controlado aleatorizado. Por el contrario, otros tratamientos sugeridos, como el drenaje transvaginal del hidrosálpinx, no ofrecen ningún beneficio. Además, el mecanismo fundamental que explique las bajas tasas de implantación y mal desarrollo embrionario aún no está definido. Conclusión: la salpingectomía laparoscópica pre-FIV se recomienda en pacientes con hidrosálpinx visible al examen ecográfico. Otros tratamientos sugeridos necesitan ser evaluados en estudios controlados aleatorizados.]]></p></abstract>

Objective: this review is aimed at evaluating the different options for treatment which can eliminate hydrosalpingueal fluid before in vitro fertilization is performed. Methodology: the literature related to the subject was selected and evaluated; data was obtained from MEDLINE and EMBASE databases between January 1997 and September 2007. The articles on the reference list were selected as the source for this review. </p>     <p><b>Key words: </b>hydrosalpinx, salpingectomy, <i>in vitro </i>fertilization (IVF). </p>     <p><b>INTRODUCCI&Oacute;N </b></p>     <p>Varios estudios retrospectivos recopilados en varios metaan&aacute;lisis han demostrado que las pacientes que presentan hidros&aacute;lpinx y son sometidas a fertilizaci&oacute;n <i>in vitro </i>(FIV), muestran una disminuci&oacute;n en la tasa de embarazo y de parto de hasta un cincuenta por ciento (50%) y tasas de aborto espont&aacute;neo dobles, comparadas con otras pacientes sin esta patolog&iacute;a, sometidas igualmente a t&eacute;cnicas de reproducci&oacute;n asistida.<sup>1-2 </sup></p>     <p>La principal teor&iacute;a detr&aacute;s de este mecanismo sugiereque el fluido del hidros&aacute;lpinx (FH)juega un papel importante como causante de estos resultados. El hidros&aacute;lpinx podr&iacute;a afectar la probabilidad de embarazo por efecto embriot&oacute;xico del FH que se filtra de manera continua a la cavidad endometrial, donde causa alteraciones endometriales haciendo hostil la implantaci&oacute;n.<sup>3-11 </sup>Por otra parte, puede producir un desprendimiento de los embriones por el efecto mec&aacute;nico de arrastre que produce la filtraci&oacute;n.<sup>12-18 </sup>Asimismo,otras teor&iacute;as han incluido simult&aacute;neamente da&ntilde;o al endometrio en el momento del inicio de la infecci&oacute;n tub&aacute;rica,<sup>19-22 </sup>da&ntilde;o de la funci&oacute;n ov&aacute;rica y calidad del oocito debido a la influencia de sustancias t&oacute;xicas inflamatorias transportadas a trav&eacute;s de la sangre o del sistema linf&aacute;tico.<sup>23-36 </sup></p>     <p>A pesar de la informaci&oacute;n existente, hasta el momento no se ha definido un esquema de tratamiento relacionado con este tema. Sin embargo, en la &uacute;ltima d&eacute;cada, numerosos estudios han demostrado su preocupaci&oacute;n acerca de la presencia de hidros&aacute;lpinx y su manejo en pacientes que van a ser sometidas a FIV. </p>     ]]></body>
<body><![CDATA[<p>Las encuestas de opini&oacute;n efectuadas a centros que implementan T&eacute;cnicas de Reproducci&oacute;n Asistida (TRA) en Europa, sobre el manejo del hidros&aacute;lpinx antes de realizar FIV, caracterizan esta situaci&oacute;n. La encuesta inglesa muestra que de 117 centros que realizan TRA, 88 centros respondieron (75%), de los cuales 80 (91%) manifestaron que ellos discutieron con las pacientes el efecto del hidros&aacute;lpinx en el resultado de la FIV. Por otra parte, 10 centros (12%) no recomendaron el tratamiento del hidros&aacute;lpinx antes de la FIV, mientras que 30 centros (36%) recomendaron el tratamiento superficialmente, 27 centros (33%) lo hicieron firmemente y otros 16 (19%) muy firmemente. Las opciones de tratamiento ofrecidas por los m&eacute;dicos tratantes fueron salpingectom&iacute;a laparosc&oacute;pica (75%), salpingectom&iacute;a por laparotom&iacute;a (45%), salpingostom&iacute;a (40%), oclusi&oacute;n tub&aacute;rica proximal (34%), aspiraci&oacute;n del l&iacute;quido del hidros&aacute;lpinx por ecograf&iacute;a transvaginal (ETV) durante aspiraci&oacute;n oocitaria (23%), y aspiraci&oacute;n ETV del fluido del hidros&aacute;lpinx antes de la aspiraci&oacute;n folicular (10%). Solamente 28% de los centros ten&iacute;an protocolos de manejo para el hidros&aacute;lpinx.<sup>37 </sup>La encuesta francesa fue realizada a 100 centros registrados en TRA, 82 centros contestaron el cuestionario, de los cuales 67% ten&iacute;an entre 10 y 20 a&ntilde;os de experiencia en infertilidad. Asimismo, 33% de las unidades de FIV ten&iacute;an un especialista en cirug&iacute;a de infertilidad y discut&iacute;an el manejo del hidros&aacute;lpinx con todo el equipo de trabajo antes de la FIV. Adem&aacute;s, 80 (98%) de los 82 centros que contestaron, indicaron que discut&iacute;an previamente los efectos del hidros&aacute;lpinx en los resultados de la FIV. De la misma forma, 6 centros (7,5%) no sugirieron un tratamiento espec&iacute;fico, mientras 12 (15%), 53 (66%), y 9 (11,3%) sugirieron un tratamiento superficialmente, firmemente, y muy firmemente respectivamente. Las opciones de tratamiento (n=171) propuestas por los m&eacute;dicos fueron: tratamiento m&eacute;dico 38 (22%), aspiraci&oacute;n del fluido del hidros&aacute;lpinx por ETV con o sin tratamiento m&eacute;dico 22 (12,9%) y tratamiento quir&uacute;rgico 111 (64,9%). &Uacute;nicamente el 11% de los centros ten&iacute;an protocolos de manejo para el hidros&aacute;lpinx.<sup>38 </sup></p>     <p>Estos resultados presentan una gran variaci&oacute;n en el manejo del hidros&aacute;lpinx antes de la FIV, con diferentes opciones de tratamiento. Muchas de estas opciones, relacionadas con la eliminaci&oacute;n del fluido del hidros&aacute;lpinx, no han sido evaluadas adecuadamente; y se considera que cualquier intervenci&oacute;n quir&uacute;rgica que interrumpa la comunicaci&oacute;n hacia el &uacute;tero podr&iacute;a prevenir el derrame de este fluido y mejorar as&iacute; las tasas de embarazo.<sup>15 </sup></p>     <p>Por consiguiente, el objetivo de esta revisi&oacute;n es aplicar las opciones de tratamiento que se discuten en la literatura pero enfoc&aacute;ndonos en aquellas que eliminan el fluido del hidros&aacute;lpinx antes de realizar una FIV. </p>     <p><b>METODOLOG&Iacute;A </b></p>     <p>Se realiz&oacute; una revisi&oacute;n de la literatura sobre el tema en las bases de datos de MEDLINE y EMBASE de los diez &uacute;ltimos a&ntilde;os, desde enero de 1997 hasta septiembre de 2007. </p>     <p>El ingl&eacute;s fue el idioma utilizado para la revisi&oacute;n, y las palabras clave utilizadas para la b&uacute;squeda en la base de datos fueron las siguientes: <i>hidros&aacute;lpinx, enfermedad tub&aacute;rica, FIV, ICSI, resultado, tratamiento, cirug&iacute;a, salpingectom&iacute;a, salpingostom&iacute;a, embarazo antes de, embarazo siguiente a y embarazo despu&eacute;s de, profilaxis, implantaci&oacute;n, t&oacute;xico, embri&oacute;n y cultivo. </i>Todas las palabras clave fueron usadas en combinaci&oacute;n con &quot;hidros&aacute;lpinx&quot;. Inmediatamente despu&eacute;s, los art&iacute;culos fueron evaluados y se les di&oacute; un puntaje con relaci&oacute;n a su objetivo, su metodolog&iacute;a, tipo de estudio y tipo de revisiones. </p>     <p><b>RESULTADOS DE LAS DIFERENTES OPCIONES DE TRATAMIENTO </b></p>     <p>En total se encontraron 126 art&iacute;culos relacionados con el tema, de los cuales se seleccionaron 54 art&iacute;culos que comprendieron estudios controlados aleatorizados, estudios observacionales y reportes de caso. </p>     <p><b>SALPINGECTOM&Iacute;A </b></p>     <p>Un estudio controlado, aleatorizado, multic&eacute;ntrico, realizado en Escandinavia compar&oacute; la salpingectom&iacute;a laparosc&oacute;pica versus no intervenci&oacute;n quir&uacute;rgica antes de un primer ciclo de FIV, demostrando una mejor&iacute;a significativa en las tasas de embarazo y de parto despu&eacute;s de una salpingectom&iacute;a en pacientes con hidros&aacute;lpinx, los cuales ya eran suficientemente grandes para ser visibles durante el examen ecogr&aacute;fico.<sup>5 </sup>La tasa de embarazo cl&iacute;nico fue de 45,7%, vs. 22,5% (p = 0,029) y la tasa de parto 40% vs. 17,5% (p = 0,038). En total, la tasa de parto aument&oacute; 3,5 veces en pacientes con hidros&aacute;lpinx bilateral, visible ecogr&aacute;ficamente despu&eacute;s de una salpingectom&iacute;a (p = 0,019). La diferencia en el resultado final no fue estad&iacute;sticamente significativa en el estudio de la poblaci&oacute;n total de 204 pacientes, el cual incluy&oacute; pacientes con hidros&aacute;lpinx que no fue visible por medio de ecograf&iacute;a, lo que sugiere que el beneficio de la salpingectom&iacute;a es &uacute;nicamente evidente si el hidros&aacute;lpinx est&aacute; lleno de fluido. </p>     ]]></body>
<body><![CDATA[<p>Una revisi&oacute;n realizada en la base de datos Cochrane en 2002 se bas&oacute; en tres estudios controlados aleatorizados, incluyendo el escandinavo, en la cual se concluy&oacute; que la salpingectom&iacute;a laparosc&oacute;pica se deber&iacute;a considerar en toda mujer con hidros&aacute;lpinx que vaya a ser sometida a FIV.<sup>39 </sup>Esta recomendaci&oacute;n tajante de salpingectom&iacute;a ha originado preocupaciones acerca de la remoci&oacute;n innecesaria de trompas uterinas que pueden ser reparadas, ya que existen argumentos a favor de realizar la microcirug&iacute;a tub&aacute;rica en pacientes con hidros&aacute;lpinx.<sup>40 </sup>La realizaci&oacute;n de la FIV debe ser una estrategia complementaria para la enfermedad tub&aacute;rica, ya que se ha demostrado que aproximadamente un tercio de las mujeres con hidros&aacute;lpinx tienen un buen pron&oacute;stico de embarazo espont&aacute;neo despu&eacute;s de cirug&iacute;a reconstructiva.<sup>41 </sup></p>     <p><b>Riesgos de complicaciones despu&eacute;s de la salpingectom&iacute;a </b></p>     <p>Se ha debatido el efecto de la salpingectom&iacute;a sobre la funci&oacute;n ov&aacute;rica, ya que hasta ahora no existen estudios concluyentes al respecto al no encontrarse una diferencia significativa en la respuesta a la estimulaci&oacute;n hormonal utilizada o la recuperaci&oacute;n oovocitaria, en los ciclos despu&eacute;s de la cirug&iacute;a. En promedio, 0,7 fol&iacute;culos menos son recuperados en comparaci&oacute;n con el ciclo previo a la intervenci&oacute;n quir&uacute;rgica.<sup>42 </sup></p>     <p>De estos resultados, no se puede concluir que las pacientes con una baja reserva ov&aacute;rica tengan un mayor riesgo de deteriorar su respuesta ov&aacute;rica despu&eacute;s de la salpingectom&iacute;a. </p>     <p><b>LIGADURA TUB&Aacute;RICA </b></p>     <p>La oclusi&oacute;n tub&aacute;rica proximal de la trompa de falopio se ha sugerido como una alternativa a la salpingectom&iacute;a, particularmente cuando se encuentran adherencias densas que dificultan de forma extrema dicho procedimiento. </p>     <p>La oclusi&oacute;n de la tuba sirve al prop&oacute;sito de interrumpir el paso del fluido a la cavidad endometrial, dejando al hidros&aacute;lpinx en su sitio, lo cual puede producir dificultad a la aspiraci&oacute;n de los oocitos. El procedimiento suele ir acompa&ntilde;ado de la realizaci&oacute;n de una ventana u orificio en la parte distal del hidros&aacute;lpinx para que &eacute;ste drene el fluido, aunque usualmente esta abertura se cierra en forma temprana. </p>     <p>En tres estudios retrospectivos, se ha encontrado que el procedimiento produce resultados comparativos a aquellos obtenidos despu&eacute;s de la salpingectom&iacute;a y mejora los resultados al ser comparado con no intervenci&oacute;n.<sup>35,43-44 </sup><a href="#Tabla1">Tabla 1</a></p>     <p>    <center><img src="img/revistas/rcog/v60n2/a08t1.jpg"><a name="Tabla1"></a></center></p>     ]]></body>
<body><![CDATA[<p>En un estudio reciente, aleatorizado y controlado en 115 pacientes con hidros&aacute;lpinx distribuidas de la siguiente manera antes de ser sometidas a FIV: 50 en ligadura tub&aacute;rica, 50 en salpingectom&iacute;a y 15 en ning&uacute;n tratamiento, ambos m&eacute;todos quir&uacute;rgicos demostraron tasas de embarazo significativamente altas (37,8 y 55,3%) comparada con las mujeres a quienes no se les realiz&oacute; el procedimiento (7,1%) analizadas con base en una intenci&oacute;n de tratamiento (p = 0,049). Aunque el estudio es de bajo poder, los resultados confirman los hallazgos de estudios previos retrospectivos, sugiriendo que la oclusi&oacute;n proximal es efectiva.<sup>45 </sup>Sin embargo, se requieren nuevos estudios para concluir que la oclusi&oacute;n proximal es tan efectiva como la salpingectom&iacute;a para incrementar la tasa de embarazo. </p>     <p>Tomando en cuenta los aspectos t&eacute;cnicos de la cirug&iacute;a en casos complicados por adherencias severas y el riesgo potencial de da&ntilde;o vascular, la ligadura proximal es una alternativa recomendada. </p>     <p><b>ASPIRACI&Oacute;N TRANSVAGINAL DEL FLUIDO </b></p>     <p>La aspiraci&oacute;n transvaginal del fluido del hidros&aacute;lpinx guiado mediante ecograf&iacute;a ha sido recomendada como una opci&oacute;n de tratamiento para extraer dicho fluido. Sin embargo, si el procedimiento es realizado antes de la estimulaci&oacute;n ov&aacute;rica, se corre un alto riesgo de que el FH se reacumule; y a&uacute;n si se realiza al momento de la aspiraci&oacute;n folicular, el riesgo de recurrencia es ya alto al momento de la transferencia. </p>     <p>En dos estudios retrospectivos de casos y controles que evaluaron el procedimiento no se encontr&oacute; ning&uacute;n beneficio.<sup>46-47 </sup><a href="#Tabla2">Tabla 2</a></p>     <p>    <center><img src="img/revistas/rcog/v60n2/a08t2.jpg"><a name="Tabla2"></a></center></p>     <p>En la situaci&oacute;n cl&iacute;nica en la cual la paciente desarrolla el fluido tub&aacute;rico durante la estimulaci&oacute;n ov&aacute;rica, la aspiraci&oacute;n transvaginal al momento de la aspiraci&oacute;n folicular puede ser una alternativa, pero la mejor opci&oacute;n es la congelaci&oacute;n de los embriones y subsiguiente cirug&iacute;a antes de transferir los embriones descongelados. Asimismo, otro fen&oacute;meno que se presenta algunas veces con el hidros&aacute;lpinx es la aparici&oacute;n de fluido en la cavidad uterina (hidr&oacute;metra). Algunos estudios retrospectivos han demostrado que la presencia de hidr&oacute;metra al momento de la transferencia, es un signo de un pron&oacute;stico pobre.<sup>48 </sup>En conclusi&oacute;n, es improbable que la aspiraci&oacute;n del fluido uterino sea beneficiosa porque hay una r&aacute;pida reacumulaci&oacute;n de hidr&oacute;metra.<sup>49 </sup></p>     <p><b>TRATAMIENTO CON ANTIBI&Oacute;TICOS </b></p>     <p>El uso de antibi&oacute;ticos ha sido presentado como un simple tratamiento para superar los efectos negativos del hidros&aacute;lpinx, basado en la teor&iacute;a de la infecci&oacute;n tub&aacute;rica persistente. El tratamiento con antibi&oacute;ticos nunca ha sido evaluado prospectivamente; sin embargo, a la fecha &uacute;nicamente un estudio retrospectivo ha sugerido que el uso de la doxiciclina durante el ciclo de la FIV podr&iacute;a minimizar el efecto delet&eacute;reo del hidros&aacute;lpinx.<sup>50 </sup></p>     ]]></body>
<body><![CDATA[<p><b>OTROS MANEJOS SUGERIDOS </b></p>     <p>En un estudio retrospectivo se ha descrito el uso de ciclo natural con la intenci&oacute;n de evitar la hiperestimulaci&oacute;n ov&aacute;rica y el subsiguiente agrandamiento del hidros&aacute;lpinx.<sup>51 </sup>En el estudio, setenta y dos pacientes con hidros&aacute;lpinx sometidas a FIV en ciclo natural mostraron una tasa de embarazo significativa, comparadas con pacientes sometidas a hiperestimulaci&oacute;n ov&aacute;rica (n = 49; 18% vs. 7%, p &lt; 0,05). Este resultado est&aacute; sesgado por la selecci&oacute;n de pacientes para los tratamientos escogidos. No obstante, el mal resultado que se obtiene en los ciclos en que se congela y descongela contradice el beneficio potencial de los ciclos naturales. </p>     <p>La sugerencia de incrementar el n&uacute;mero de embriones a transferir para contrarrestar el efecto adverso del hidros&aacute;lpinx no ha demostrado ser efectiva; por lo tanto, no se deber&iacute;a considerar, ya que en el momento actual los esfuerzos se dirigen a disminuir la tasa de embarazo m&uacute;ltiple, y la tendencia es la de transferir un s&oacute;lo embri&oacute;n. </p>     <p><b>CONCLUSIONES </b></p>     <p>Las pacientes con hidros&aacute;lpinx que van a ser sometidas a FIV tienen pobre pron&oacute;stico ya comprobado. Su influencia negativa a&uacute;n no se ha comprendido completamente, y es un &aacute;rea que necesita m&aacute;s investigaci&oacute;n. No obstante, se ha demostrado que la salpingectom&iacute;a laparosc&oacute;pica pre-FIV es efectiva en restaurar las tasas de nacimiento y debe recomendarse si el hidros&aacute;lpinx es visible en el examen ecogr&aacute;fico. La ligadura o secci&oacute;n de la tuba a nivel &iacute;stmico es una alternativa conveniente si la salpingectom&iacute;a es dif&iacute;cil de realizar.
