Presentamos    el caso de un hombre de 56 a&ntilde;os, matarife, con cuadro de dolor abdominal, document&aacute;ndose    por TAC y en cirug&iacute;a dilataci&oacute;n aneurism&aacute;tica de la aorta por debajo de las    renales, con colecci&oacute;n purulenta y ulceraci&oacute;n del vaso de donde se aisl&oacute; Streptococcus    equi zooepidemicus, aisl&aacute;ndose tambi&eacute;n la bacteria de los hemocultivos.</p>     <p>Palabras clave: aortitis, S. equi zooepidemicus.</p> <hr size="1">     <p><font size="3"><b>Abstract</b></font></p>     <p>Infections by Streptococcus equi subspecies zooepidemicus occur in animals. En los equinos    la entidad m&aacute;s reportada es &quot;el estrangulamiento&quot;, con cuadro cl&iacute;nico caracterizado    por fiebre de inicio abrupto seguido de compromiso de tracto respiratorio superior,    secreci&oacute;n mucopurulenta por nariz, formaci&oacute;n de abscesos submandibular y retrofar&iacute;ngeo    y adenopat&iacute;as en cuello. Otros sitios frecuentes de infecci&oacute;n son pulm&oacute;n, bazo,    ri&ntilde;&oacute;n y cerebro; de 15 casos reportados con estrangulamiento, cinco ten&iacute;an neumon&iacute;a    o pleuroneumon&iacute;a. Puede afectar tambi&eacute;n cabras, ovejas, ganado vacuno, aves    de corral y cerdos (2), produciendo adem&aacute;s endometritis, mastitis, linfadenitis,    sepsis bovina (3). En Canad&aacute;, de 1.323 aislamientos bacterianos provenientes    de equinos, la bacteria m&aacute;s frecuentemente aislada result&oacute; ser el Streptococcus    equi zooepidemicus (4). </p>     <p>Se ha reportado infecci&oacute;n en humanos consider&aacute;ndose una zoonosis de origen    profesional, en manipuladores de animales, veterinarios o estudiantes de veterinaria,    pero no hace parte de la flora normal del hombre (3)</p>     <p>El Streptococcus del grupo C tiene dos especies con dos subespecies cada una:    dysgalactie subespecie dysgalactie y dysgalactie subespecie equisimilis (el    m&aacute;s frecuente en humanos, y de donde se obtiene la estreptoquinasa) equi subespecie    equi y equi subespecie zooepidemicus (el m&aacute;s agresivo) (5,6). </p>     ]]></body>
<body><![CDATA[<p>Presentamos el caso de un hombre de 56 a&ntilde;os, profesi&oacute;n matarife, que ingres&oacute;    el mes de marzo 2008 por dolor intenso en mesogastrio irradiado a espalda de    cinco d&iacute;as de evoluci&oacute;n, asociado a astenia y adinamia. El examen f&iacute;sico totalmente    normal, incluyendo la palpaci&oacute;n abdominal sin defensa ni masas. Los ex&aacute;menes    de laboratorio mostraron leucocitosis de 12.500 con neutrofilia de 71% y sedimentaci&oacute;n    de 120 mm. La TAC de abdomen mostr&oacute; dilataci&oacute;n aneurism&aacute;tica de la aorta por    debajo de las renales con colecci&oacute;n paraa&oacute;rtica (<a href="img/revistas/amc/v34n2/a6f1.jpg">Figura    1</a>). Se llev&oacute; a cirug&iacute;a encontr&aacute;ndose aneurisma de 4 cm en la aorta infrarrenal    con colecci&oacute;n purulenta a su alrededor y cuatro &uacute;lceras en la media y muscular;    el cultivo del material purulento y los hemocultivos mostraron crecimiento de    Streptococcus equi variedad zooepidemicus. Se practic&oacute; drenaje y luego colocaci&oacute;n    de injerto con tres procedimientos quir&uacute;rgicos y evoluci&oacute;n t&oacute;rpida hasta 50    d&iacute;as despu&eacute;s del diagn&oacute;stico cuando presenta enterorragia masiva y fallece (probable    f&iacute;stula aortoent&eacute;rica). </p>     <p><font size="3"><b>Discusi&oacute;n</b></font></p>     <p>El Streptococcus equi zooepidemicus es de todos los Streptococcus C el m&aacute;s    agresivo en el humano, se han descrito brotes de esta infecci&oacute;n y casos espor&aacute;dicos    severos. Las infecciones est&aacute;n muy frecuentemente asociadas a bacteriemia: (1,    7) en una casu&iacute;stica de 31 pacientes, 74% cursaron con bacteriemia (8). Como    reacci&oacute;n a distancia pueden producir glomerulonefritis postestreptoc&oacute;ccica pero    no eleva las ASTOS pues el S. equi zooepidemicus no las produce (9-11). En un    reporte de 15 casos de bacteriemia, se presentaron cuatro casos de nefritis    (12). La mortalidad en casos de bacteriemia llega a 40% (1). </p>     <p>En humanos son pocos los casos descritos de infecci&oacute;n por este microorganismo:    meningitis, neumon&iacute;a, infecci&oacute;n de tejidos blandos, artritis, sepsis. (12-16).    Los casos reportados de endocarditis e infecci&oacute;n endovascular son s&oacute;lo 21, el    porcentaje de pacientes con aneurisma a&oacute;rtico (26.7%) es elevado (16), las infecciones    endovasculares est&aacute;n asociadas a drogadicci&oacute;n por v&iacute;a IV e infecci&oacute;n de aneurismas    arterioscler&oacute;ticos como el caso descrito. En Hong Kong, de 11 casos con bacteriemia    por Streptococcus equi zooepidemicus, tres ten&iacute;an infecci&oacute;n vascular, dos de    ellos con aneurisma a&oacute;rtico infectado por este germen; y de 45 casos con bacteriemia,    tres ten&iacute;an infecci&oacute;n de aneurisma a&oacute;rtico, falleciendo dos de ellos (16). Cinco    de los 15 casos reportados por Bradley ten&iacute;an endocarditis (12), por ello se    recomienda en casos de bacteriemia por Streptococcus equi zooepidemicus buscar    siempre infecci&oacute;n endovascular no s&oacute;lo en coraz&oacute;n, sino tambi&eacute;n en grandes vasos    (1, 17). </p>     <p>La infecci&oacute;n en el humano se da por contacto estrecho con animales dom&eacute;sticos,    por el consumo de leche no pasteurizada y aun por la ingesta de carne de cerdo    (1, 2, 18). La ruta de transmisi&oacute;n en la mayor&iacute;a de los casos no ha sido claramente    identificada, pero incluye v&iacute;a subcut&aacute;nea por cicatrices, &uacute;lceras, drogadicci&oacute;n,    por v&iacute;a de tracto respiratorio superior y por tracto gastrointestinal (19).    Se report&oacute; en Serbia una mujer con meningitis por S equi zooepidemicus y el    mismo germen se aisl&oacute; en sus dos caballos (20), la v&iacute;a de infecci&oacute;n puede ser    accidental como en el caso de nuestro paciente que trabajaba como matarife en    un centro de sacrificado de animales. </p>     <p>El tratamiento se hace con penicilina, aunque se han informado cepas tolerantes;    en casos severos como el anotado, se recomienda buscar el efecto sinergico de    la gentamicina (21, 22).</p>     <p><font size="3"><b>Referencias</b></font></p>     <!-- ref --><p> 1.
