Fundaci&oacute;n Santa Fe de Bogot&aacute;</p>  <hr size="1">     <p><b><font face="Verdana" size="2"><font face="Verdana" size="2"><font face="Verdana" size="2"></font></font></font><font size="3">CASO CL&Iacute;NICO</font></b>       <P> Paciente de 64 a&ntilde;os con cuadro de trastorno linfoproliferativo   relacionado a tratamiento inmunosupresor   por trasplante hep&aacute;tico realizado 2 a&ntilde;os antes, presenta sepsis secundaria a infecci&oacute;n de la herida quir&uacute;rgica, por lo cual ingresa a la unidad de cuidado intensivo requiriendo posteriormente intubaci&oacute;n   orotraqueal. Asociado a esto y a trombosis de la vena porta, presenta pancitopenia, con trombocitopenia   que requiri&oacute; transfusiones por sangrado   en m&uacute;ltiples ocasiones, llegando hasta valores de 3.000 plaquetas mm<SUP>3</sup>. El paciente necesit&oacute; ventilaci&oacute;n mec&aacute;nica prolongada por lo cual el d&iacute;a 17 se decide programar para traqueostom&iacute;a percut&aacute;nea.</P>     <P>   <b>Descripci&oacute;n del procedimiento</b></P>     <P>   Se realiz&oacute; procedimiento con t&eacute;cnica de traqueostom&iacute;a   percutanea mediante anestesia endovenosa   con Propofol, Fentanyl y Cisatracurium como relajante muscular. Para el manejo de la v&iacute;a a&eacute;rea, mientras se retir&oacute; el tubo para la realizaci&oacute;n de la ruta de la traqueostom&iacute;a se utiliz&oacute; el intercambiador   de Frova a trav&eacute;s del tubo endotraqueal. Con el tubo orotraqueal ascendido a la glotis y el intercambiador colocado infragl&ograve;tico se suministr&oacute; ventilaci&oacute;n manual durante todo el tiempo del procedimiento,   lo cual produjo que el paciente nunca se desataurara y el transoperatorio fuera totalmente   tranquilo. Una vez comprobada y asegurada la v&iacute;a a&eacute;rea con la traqueostom&iacute;a se retir&oacute; el intercambiador   de Frova y se conect&oacute; nuevamente el paciente al ventilador a trav&eacute;s de la traqueostom&iacute;a realizada exitosamente.<a href="#(fig1)"> Fig.1</a>.</P>      <p>        <center>  <a name="(fig1)"><img src="img/revistas/rca/v36n1/v36n1a10fig1.gif"></a> </center> </p>     <P>  <b><font size="3">DISCUSI&Oacute;N</font></b></P>     <P>   La traqueostom&iacute;a es un procedimiento muy com&uacute;n   en las unidades de cuidado intensivo, present&aacute;ndose   con una incidencia de 15-20%, de las cuales   muchas se realizan de manera percut&aacute;nea por ser un procedimiento m&aacute;s costo-efectivo. La traqueostom&iacute;a   percutanea es una t&eacute;cnica ciega que utiliza una aguja, gu&iacute;a, dilatadores y tiene el riesgo de desinflar el neumotaponador, lesionar el tubo y provocar una extubaci&ograve;n accidental, por lo que de esta manera puede comprometerse la v&iacute;a a&eacute;rea<sub>(1,2)</sub>.</P>     ]]></body>
<body><![CDATA[<P>   La incidencia de complicaciones perioperatorias de la traqueostom&iacute;a percut&aacute;nea en estudios prospectivos   es del 10% e incluye sangrados, enfisema, neumot&oacute;rax, colocaci&oacute;n inadecuada de la c&aacute;nula de traqueostom&iacute;a y dilatadores, p&eacute;rdida accidental de la v&iacute;a a&eacute;rea y una variedad de complicaciones inmediatas y a largo plazo<sub>(2,3)</sub>.</P>     <P>   Se han investigado diversos m&eacute;todos para mantener   la ventilaci&oacute;n pulmonar durante el procedimiento,   entre estos se encuentra la m&aacute;scara laringea   (LMA), el combitubo, la m&aacute;scara laringea intubadora (fastrach), la m&aacute;scara laringea Proseal y el broncoscopio r&iacute;gido<sub>(4,5,6,7,8)</sub>.</P>     <P>   Verghese et. al. report&oacute; con &eacute;xito un estudio piloto   de 10 pacientes en donde se utiliz&oacute; la m&aacute;scara laringea intubadora (fastrach), la cual contaba con un tubo endotraqueal y el fibroscopio por si hab&iacute;a la necesidad de reintubar al paciente, esta t&eacute;cnica fue efectiva en el 100% de los pacientes<sub>(9,10)</sub>.</P>     <P>   Bilotta y colaboradores recomendaron el uso de un intercambiador de tubo para guiar la inserci&oacute;n de la m&aacute;scara laringea ya que existe un 5 % de los casos en donde por medio de la fibrobroncoscopia a trav&eacute;s de la m&aacute;scara laringea no se visualiza claramente   la traquea<sub>(11)</sub>.</P>     <P>   Mallick y colaboradores realizaron un estudio observacional prospectivo en donde se utilizaba el combitubo para la realizaci&oacute;n de traqueostom&iacute;a   percutanea, en 21 pacientes donde la t&eacute;cnica fue efectiva en 20 pacientes, aunque solo se logro ventilar adecuadamente a 17 pacientes (85%) del total<sub>(12)</sub>.</P>     <P>   Jefferson et. al. describi&oacute; la t&eacute;cnica de utilizaci&oacute;n   de un fibroscopio r&iacute;gido para la realizaci&oacute;n de una traqueostom&iacute;a percut&aacute;nea en una mujer, mostrando la ventaja de su utilizaci&oacute;n ya que permite la observaci&oacute;n continua de todas las etapas del procedimiento y la posibilidad de ventilar a trav&eacute;s   de un sistema inyector<sub>(13)</sub>.</P>     <P>La traqueostom&iacute;a percutanea tambi&eacute;n se ha realizado en la poblaci&oacute;n obesa y se ha encontrado que estos pacientes tienen 5 veces m&aacute;s probabilidades   de complicaciones perioperatorias que la poblaci&oacute;n control y de &eacute;stas la perdida del control de la v&iacute;a a&eacute;rea tiene una incidencia de 27%<sub>(14,15)</sub>.</P>     <P>   Todos los m&eacute;todos que se han utilizado para el mantenimiento de la v&iacute;a a&eacute;rea durante el procedimiento   han tenido &eacute;xito, sin embargo ninguno esta exento de complicaciones debido a que no protegen   de la aspiraci&oacute;n de contenido g&aacute;strico, p&eacute;rdidad   de la v&iacute;a a&eacute;rea y en pacientes con la distensibilidad pulmonar disminuida no permiten una adecuada ventilaci&oacute;n, adicionalmente al colocar una m&aacute;scara laringea o el combitubo tambi&eacute;n se puede distorsionar la anatom&iacute;a del cuello haciendo m&aacute;s dif&iacute;cil la t&eacute;cnica ciega para la traqueostom&iacute;a percutanea<sub>(16,17,18,19)</sub>.<a href="#(fig2)"> Fig. 2</a>.</P>     <p>        <center>  <a name="(fig2)"><img src="img/revistas/rca/v36n1/v36n1a10fig2.gif"></a> </center>  </p>    ]]></body>
<body><![CDATA[<P>  En este caso se utiliz&oacute; un intercambiador de Frova,   con el cual es posible tener asegurada la v&iacute;a a&eacute;rea durante todo el procedimiento, porque el dispositivo siempre est&aacute; intratraqueal, sin tener necesidad de retirarlo ni siquiera cuando se avanza la c&aacute;nula de traqueostomia dentro de la traquea. Adem&aacute;s de poder   realizar bien sea ventilaci&oacute;n jet o manual a trav&eacute;s de &eacute;l, permitiendo de esta manera una ventilaci&oacute;n y una oxigenaci&oacute;n adecuada del paciente en todo momento. El intercambiador de Frova a diferencia de otros instrumentos de v&iacute;a a&eacute;rea, utilizados para este procedimiento, no deforma la anatom&iacute;a del cuello, lo que facilita el acceso quir&uacute;rgico para el cirujano, logr&aacute;ndose un abordaje a la traquea con rapidez.
