Todos los  derechos reservados.</p> <hr>     <p>A prop&oacute;sito del art&iacute;culo de Reflexi&oacute;n &laquo;La lesi&oacute;n por almacenamiento y la transfusi&oacute;n sangu&iacute;nea&raquo;<sup>1</sup>, realizado sobre el art&iacute;culo &laquo;T&eacute;cnicas de ahorro sangu&iacute;neo en cirug&iacute;a&raquo;   <sup>2</sup>,donde se plantea que la sangre de banco sufre alteraciones por   &laquo;almacenamiento&raquo;, llevando a peores desenlaces con unidades trasfundidas   &laquo;viejas&raquo;, cabe resaltar que estudios con desenlaces intermedios, como   disfunci&oacute;n pulmonar, no han encontrado diferencias estad&iacute;sticas al   trasfundir sangre con diferente tiempo de almacenamiento, especialmente   en la UCI<sup>3</sup><sup>, </sup><sup>4</sup>;   otros estudios s&iacute; reportan alteraciones, especialmente en trasfusi&oacute;n   masiva y trauma. Aunque persista la controversia, es recomendable   transfundir sangre lo menos vieja posible; alguna bibliograf&iacute;a que   enriquece el tema e involucra otros hemoderivados como las plaquetas es   citada al final<sup>5</sup><sup>, </sup><sup>6</sup><sup>, </sup><sup>7</sup><sup>, </sup><sup>8</sup><sup>, </sup><sup>9</sup>.</p>      <p>Sobre los niveles de Hb de 6 y 10g/dl, no hay consenso para tomarlos   taxativamente para definir una transfusi&oacute;n. Sin embargo, al cl&iacute;nico hay   que darle herramientas para precisar la situaci&oacute;n ante el paciente, y   por tanto me parece adecuada la frase tomada de la Asociaci&oacute;n Americana   de Anestesi&oacute;logos (ASA Task Force): &laquo;Esta decisi&oacute;n entre 6-10g/dl debe   ser individual y basada en par&aacute;metros cl&iacute;nicos y paracl&iacute;nicos que   demuestren hipoperfusi&oacute;n tisular&raquo;<sup>5</sup>.   Es decir, al valor de Hb hay que sumarle los signos de hipoperfusi&oacute;n   tisular, y esta ser&iacute;a una recomendaci&oacute;n razonable y pr&aacute;ctica, si bien   puede ser a&uacute;n discutida.</p>     ]]></body>
