A raíz de la publicación de la declaración de consenso acerca de la disfunción tiroidea subclínica en el año 2005, realizada por la Sociedad Americana de Endocrinologìa (The Endocrine Society), la Asociación Americana de Endocrinólogos Clínicos (AACE) y la Asociación Americana de la Tiroides (ATA) se abrió un nuevo espacio para los endocrinólogos en el manejo de algunas disfunciones tiroideas que no habían sido tenidas en cuenta dentro del campo de accion terapéutico. Los autores hacen un recuento de cómo se produce el cambio en los paradigmas y en el enfoque diagnostico y terapéutico. Luego hacen un recuento de algunas de las experiencias desarrolladas en Colombia. Finalmente sugieren algunos derroteros a partir de su propia experiencia y también basados en logros científicos de otras fuentes obtenidos en otros campos de la Endocrinologìa moderna.]]></p></abstract>

With the publication of the Consensus Declaration about subclinical thyroid dysfunction by the Endocrine Society, the American Association of Clinical Endocrinologists (AACE), and the American Thyroid Association (ATA), a new field was open for endocrinologist in the management of some long ignored thyroid dysfunctions that did not have any therapeutic solution. The authors describe the paradigm shift and the changes in the diagnostic and therapeutic approaches. Later they recount some experiences developed in Colombia, S.A. Finally, they suggest some guidelines from their own experience and from other sources obtained in different fields of modern endocrinology.]]></p></abstract>

<body><![CDATA[<br> <b>Fecha de recepci&oacute;n:</b> 5 de mayo de 2007    <br> <b>Fecha de aceptaci&oacute;n:</b> 19 de junio de 2007    <br></font></p>     <br><hr>      <p align="left"><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A ra&iacute;z de la publicaci&oacute;n de la Declaraci&oacute;n de Consenso acerca de la disfunci&oacute;n Tiroidea Subcl&iacute;nica publicada tiempo atr&aacute;s, en el 2005 (<a href="#1">1</a>), fue evidente el desacuerdo entre este consenso y el obtenido en la Conferencia de Manejo realizada en septiembre del 2002 (<a href="#2">2</a>). Se detectaron tres &aacute;reas grises de desacuerdo, puesto que en el 2002 no se recomendaba: 1) prueba de tamizaje rutinario para hipotiroidismo subcl&iacute;nico en la poblaci&oacute;n general, 2) tamizaje rutinario en busca de hipotiroidismo subcl&iacute;nico para mujeres que se encuentran en embarazo o que planean embarazarse, y 3) tratamiento rutinario de pacientes con hipotiroidismo subcl&iacute;nico que presenten niveles de TSH de 4.5 a 10 mU/ litro. Este cambio fue claramente explicado y analizado.</font></p>      <p align="left"><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Este nuevo consenso estuvo basado en el principio de que ausencia de evidencia no es evidencia de ausencia y sigue los preceptos de la medicina basada en evidencia (MBE), en la cual las recomendaciones deben sustentarse en buena evidencia, luego en evidencia aceptable y finalmente en la opini&oacute;n de los expertos. Por lo tanto, este &uacute;ltimo consenso avala el tamizaje rutinario del hipotiroidismo subcl&iacute;nico en la poblaci&oacute;n general y en las mujeres durante la evaluaci&oacute;n preembarazo o una vez que el embarazo ha sido diagnosticado. Asimismo, recomienda la medici&oacute;n de anticuerpos antiperoxidasa (anti-TPO) como un valioso recurso adicional en la evaluaci&oacute;n de los pacientes con hipotiroidismo subcl&iacute;nico. Finalmente, el consenso aconseja que la mayor&iacute;a de pacientes con hipotiroidismo subcl&iacute;nico y algunos de aquellos con hipertiroidismo subcl&iacute;nico deben ser tratados. Estuvieron de acuerdo con el consenso anterior en que mientras no se disponga de mayor evidencia cient&iacute;fica, la mejor atenci&oacute;n m&eacute;dica contin&uacute;a siendo el combinar &quot;el juicio cl&iacute;nico con las preferencias del paciente&quot; (<a href="#2">2</a>).</font></p>      <p align="left"><font face="Verdana, Arial, Helvetica, sans-serif" size="2">El hipotiroidismo subcl&iacute;nico ha sido una preocupaci&oacute;n de los cl&iacute;nicos y de los tiroid&oacute;logos por m&aacute;s de 65 a&ntilde;os (<a href="#3">3</a>). Despu&eacute;s del descubrimiento de la hormona liberadora de tirotropina (TRH) (<a href="#4">4</a>, <a href="#5">5</a>) se propuso una nueva clasificaci&oacute;n de hipotiroidismo basada en la respuesta de la tirotropina (TSH) al TRH (<a href="#6">6</a>) y las publicaciones acerca de hipotiroidismo subcl&iacute;nico se incrementaron sustancialmente (<a href="#7">7-12</a>). La controversia acerca de este tema no ha sido nueva (<a href="#13">13</a>).</font></p>      <p align="left"><font face="Verdana, Arial, Helvetica, sans-serif" size="2">En Colombia (Suram&eacute;rica), las cosas no han sido diferentes. Conocida como un &aacute;rea bociog&eacute;nica desde la &eacute;poca de la Colonia en el siglo XVIII (<a href="#14">14</a>, <a href="#15">15</a>), Colombia fue el primer pa&iacute;s del mundo que intent&oacute; oficialmente yodizar la sal bajo la supervisi&oacute;n del cient&iacute;fico franc&eacute;s Jean Baptiste Boussingault, evento que termin&oacute; en una intoxicaci&oacute;n masiva con yodo en 1830 (<a href="#16">16</a>). La yodaci&oacute;n de la sal comenz&oacute; en nuestro pa&iacute;s en 1951 (<a href="#17">17</a>).</font></p>      <p align="left"><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Los estudios de hipotiroidismo subcl&iacute;nico comenzaron en Colombia en 1978 con el advenimiento de las pruebas provocativas con TRH (<a href="#18">18</a>, <a href="#19">19</a>). Varios estudios fueron publicados posteriormente (<a href="#20">20-27</a>). Para nuestra sorpresa, la suplementaci&oacute;n de yodo en la ciudad de Barranquilla fue encontrada deficiente (<a href="#28">28</a>, <a href="#29">29</a>), tendencia que parece estar ocurriendo tambi&eacute;n en otras partes (<a href="#30">30</a>, <a href="#31">31</a>). Esta deficiencia de yodo puede tener m&uacute;ltiples implicaciones para la salud p&uacute;blica. Cuando esta deficiencia es severa, las repercusiones cl&iacute;nicas son indiscutibles, y por lo tanto no son objeto de este art&iacute;culo. Sin embargo, en deficiencias leves o moderadas, las consecuencias son m&aacute;s sutiles, y una de ellas es el hipotiroidismo subcl&iacute;nico, el cual abarca una variedad de consecuencias cl&iacute;nicas que transcienden varias especialidades m&eacute;dicas (<a href="#32">32-40</a>). El retardo en el crecimiento y la baja talla pueden ser otra consecuencia del mismo problema y el tratamiento con hormonas tiroideas una alternativa a la suplementaci&oacute;n con yodo para ayudar a los ni&ntilde;os a recuperar su talla normal (<a href="#41">41</a>). Existe informaci&oacute;n cient&iacute;fica que sugiere que el verdadero rango de normalidad para el TSH es mucho m&aacute;s estrecho que el valor aceptado hasta hoy en d&iacute;a (<a href="#42">42-45</a>), y se ha llegado a proponer 3 mU/l como l&iacute;mite superior normal. Tambi&eacute;n se ha planteado que debido a la amplia variaci&oacute;n de los valores de hormonas tiroideas entre distintos individuos, en contraposici&oacute;n al estrecho margen de variabilidad dentro de cada individuo (<a href="#46">46-49</a>), el diagn&oacute;stico de las disfunciones tiroideas depende en forma considerable de la posici&oacute;n en que se encuentra el punto de ajuste hormonal para T3 y T4 del individuo dentro de los rangos de referencia del laboratorio utilizado (<a href="#49">49</a>).</font></p>      <p align="left"><font face="Verdana, Arial, Helvetica, sans-serif" size="2">La experiencia obtenida con la diabetes mellitus de que los valores obtenidos a trav&eacute;s de las pruebas basales y de las pruebas provocativas (en este caso la prueba de tolerancia a la glucosa o curva de glicemia) no necesariamente detectan al mismo tipo de disfunci&oacute;n metab&oacute;lica y, por lo tanto, a los mismos individuos (<a href="#50">50</a>, <a href="#51">51</a>), nos deber&iacute;a hacer pensar en la posibilidad de agregar herramientas adicionales al armamentarium diagn&oacute;stico de las disfunciones tiroideas, algo que era muy bien conocido en el pasado (<a href="#52">52</a>). Quiz&aacute; exista un papel para la prueba de TRH despu&eacute;s de todo. Aquellos que no conocen el pasado est&aacute;n condenados a repetirlo.</font></p>     ]]></body>
